PET-CT case of the month (05/09): Synchronous Squamous Cell Cancers

A 64 yrs old male presented with an exophytic lesion on the right foot 4th
interdigital web space, and was found to have SCC on punch biopsy. Staging
CT scan showed a mass in the epiglottis, measuring 2.0 cm x 1.6 cm in size.
Biopsy under direct laryngoscopy revealed SCC.
Whole body PET-CT showed focal hypermetabolism in the epiglottis (SUV 9.0)
and focal hypermetabolism in a 5mm endobronchial soft tissue density at the
trifurcation of the basal trunk of the left lower lobe bronchus (SUV 3.9).
Endoscopic biopsy showed poorly differentiated SCC and led to the management
of three synchronous primary SCCs.
